Please ensure that you have indicated what symptoms you have so that we may better understand the purpose of your visit. Keep in mind that this is an appointment request form, and an appointment cannot be guaranteed on a requested date if the clinic is already fully booked. In that case, the staff will kindly discuss other potential appointment dates with you when they call to reply to your inquiry.

You can expect a call from us within 24 hours of submitting an appointment request. If you do not receive a call within 24 hours during business days, please call our clinic. We also ask patients with phone numbers outside of Japan to call us.
